Dr. Barry Barmann's Registration Forms can be seen below. Welcome to our Clinic, and I want you to know how much I look forward to working with you. Seen below are the only forms you need to complete, in order to begin your treatment at our Anxiety Disorders Treatment Center.  I strongly reccommend that you download these forms now, fill them out, and bring them with you to your initial session.  Should you leave them at home, no "worries", I'll have copies waiting for you once you arrive. So, first, open the PDF file entitled "HIPAA Notice of Privacy Practices".  Please read it, and then download and sign the PDF document entitled, "Receipt of Notice of Privacy Practices". Be sure to bring this document with you.  Finally, download, print out and sign the Adult or Minor Intake Registration Form, and bring this with you as well.  That's it!  Looking forward to meeting you!
